Sub: Central Government Employees Group Insurance Scheme, 1980 (CGEGIS) – Pasting the Form No. 13 in the Service Book of Government employees admitted to the scheme.

1. The undersigned is directed to invite attention of all Ministries/ Departments of the Central Government to Para (6) of the OM No.S.11013/2/81/TA/2907, dated 22.09.1981 of Ministry of Finance, Department of Expenditure, Controller General of Accounts which lays down inter alia that in the event of transfer of employee from one Ministry/ Department/ Office to another Ministry/ Department/ Office the orders of transfers should clearly indicate the Group to which the transferred employee belongs and the date of his/ her continuous membership in that Group and in lower Group, if any, for the benefits from the Insurance Fund or the Insurance Fund and the Savings Fund as the case may be, under the Central Government Employees Group Insurance Scheme (CGEGIS), 1980. Further, the Heads of Office are required to endorse to the transferring D.D.O. and the Pay and Accounts Officer, a copy of order of transfer. On receipt of the order of transfer, the D.D.O. concerned with the employee prior to his/ her transfer is required to incorporate a certificate on the L.P.C. to the effect that the individual concerned had a regularly subscribed to the Insurance Fund or both the Insurance Fund and the Savings Fund up to and including the month of transfer etc.
2. It has been noticed that the above instructions are not adhered to strictly by the number of Central Ministries/ Departments. Non-observance of the prescribed procedure in this regard would lead to delay in effecting recoveries of subscription and making the payment due under the scheme causing thereby considerable inconvenience to the beneficiaries. To obviate any possible hardship it is once again impressed upon all the Ministries/ Departments/ Office then necessity to follow scrupulously the instructions laid down and C.G.A.’s aforesaid OM, dated 22.09.1981.
3. In order to further streamline the procedure in this regard, it has been decided that sufficient copies of Form No. 13, as in the proforma appended to this O.M. should invariably be included in the Service Books of all the existing members as well as the new members admitted to the Scheme hereafter. Every year, in the month of January and at the time of transfer of the member of the scheme, the Head of Office shall record a certificate as given below, in the remarks Column (Col. No. 7) of the Form No.13 over his dated signatures:
“Subscription at the rate of Rs. ………….. appropriate to Group ……….. of the scheme recovered from pay and allowance for the period from January ………. to December ……………”
4. All other events in the nature of promotion, transfer on Deputation/ Foreign Service, absorption in Public Sector Undertakings/ Autonomous Bodies, retirement etc. occurring during the service career of the member of the Scheme, shall also be recorded in the appropriate Column (Col. No.6) of Form No.13 and duly attested by the appropriate authority over his dated signatures.
5. Insofar as the persons working in the Indian Audit and Accounts Department are concerned, these orders issued in consultation with the Comptroller and Auditor General of India.
