A copy of OM No.F.3(2)-E.IV(A)/73, dated 11.03.1976 & No.F.3(2)-E.IV(A)/76, dated 06.10.1976 together with their enclosures regarding revised form of service book received from the Ministry of Finance, (Department of Expenditure) was sent along with Board’s letter under reference for introduction of the same on the Railways. Column 6 of the Revised Form of the Service Book provides for insertion of date of birth of the employees by Christian era and wherever possible also and Saka era, both in words and figures.
It has however come to notice of this department that on one of the Zonal Railways, the date of birth of an employee had at the time of his entry into service, been entered in the service records in numericals only and not in words also. On account of this lacuna, at a later date, the employee concerned, with the connivance of the dealing stuff, got the year of his birth altered to his advantage. Since the non-recording of the date of birth in words along with numericals has resulted in tempering with the recorded date of birth, recorded in numerical only, it is requested that necessary instructions may be issued to all concerned to the effect that entries regarding date of birth in the Service Book of the officials at the time of their entry into service should invariably be made both in words and in figures as already provided for in Column 6 of the Revised Form of Service Book referred to above.
